Need help starting the assessment

  • To start your assessment, follow the link in your SMS or email invite. You can also start it by going to https://patient.ultramed.app and entering your 12 character access code.

  • You can find your code at the end of the link sent to you via SMS or email. For example, if your link is https://patient.ultramed.app/c/ABCDEFGHIGKL, your code is ABCDEFGHIJKL.

  • Only the latest link will be valid. Please use the link or a code from the link from the most recent text/email you have received. If that doesn’t work, please get in touch with our support team.

  • Your hospital might issue your access code a little later to ensure that the letter arrives first. Please refer to any information in the letter provided by the Trust. Alternatively, please get in touch with our support team.

  • When entering your date of birth the format is DD/MM/YYYY, for example someone born on the 26 January 1990 would enter 26 01 1990.

  • You don’t need an email address to access the questionnaire. You should have received a link with a 12 character code, which you can enter here at https://patient.ultramed.app

Need help completing the assessment

  • Yes, if you need to leave the online questionnaire, all of your progress will be saved.

  • You can log back in via the invite link sent to you via SMS or email. Alternatively, go to https://patient.ultramed.app and enter the code and your date of birth. You will find the instructions to locating your code on the login page.

  • Once submitted, you can no longer access your assessment without your hospital’s permission. If you would like to make some changes, please get in touch with your Trust.

  • You will see a confirmation on your screen once you submit your assessment. If you would like to confirm that you have successfully submitted your assessment please get in touch with our support team.

  • If you would like to request a copy of your answers, please get in touch with our support team.

  • You can check the name of your Trust/hospital in the first invite message. It will be mentioned in the first line of the text: Your [Trust name] team need you to complete an assessment before your procedure.

  • Only your hospital will know the type and name of your procedure.

  • For any appointment or procedure questions, please reach out to your hospital team.

  • These questions are used to assess mobility. Gardening, mopping the floor and other suggestions in the questionnaire are various forms of strenuous activities that can help the hospital assess your mobility.

  • Your Trust/hospital might require this information in case it will impact your procedure or aftercare. We understand that not everyone might be comfortable providing this information, therefore it is optional.

  • To edit information about your medication, go to the “My Medications” section of the questionnaire. In the list of the entered medications, find the one you would like to amend and click on “Change”. You can amend the name, how you take the medication, dosage and frequency of taking it. Once amended, click “Update”.

  • You need to answer at least 80% of the questions across all the sections to be able to submit the questionnaire. Please check that you answered questions in all sections, even those unrelated to your procedure.

  • If you are using a mobile device it may help to use a laptop or a tablet to complete the assessment instead. If you are using a laptop, you may need to zoom out. You can do this by clicking the settings menu in the top right hand corner of the screen (three dots). From here there should be an option to zoom out.

  • The questionnaire may contain links to additional information about your procedure that may take you to a different page. If this opens in a new tab, your questionnaire should still be available in your web browser. This may look like a little blue U at the top of your screen if you are using a laptop. Alternatively, you can open a new tab and log in again at https://patient.ultramed.app/. This doesn’t mean you will have to start again as your progress through the assessment saves automatically.

  • If you are able to specify which leaflets you would like, our support team can email you the links.

Cannot complete the assessment

  • If you cannot fill out an online assessment, please reach out to your hospital, they will be able to make alternative arrangements for you.
